With international development program cycles often having a “project design phase”, how can online learning as a team improve project design?

How do you design a technology program intervention to improve health outcomes?

HIV Challenges and Keeping Up with mHealth

According to the WHO, HIV has claimed 39 million lives so far globally with 1.5 million lives in 2013 alone. At the end of 2013, there were 35 million people living with HIV, with 2.1 million becoming newly infected. With 24.7 million people living with HIV in 2013, Sub-Saharan Africa is the most affected region in the world accounting for almost 70% of the global HIV infections.

HIV often gets highlighted as a major problem in Sub-Saharan Africa, but it is also a major public health concern in Southeast Asia, particularly in Vietnam where the use of needles to inject drugs drives the epidemic. As of 2012, 260,000 (of the 89 million) people in Vietnam are living with HIV, according to UNAIDS Vietnam. As a result, FHI 360 is working with the Government of Vietnam to address the country’s HIV challenges with “effective programs that cost less, are implemented locally, and decrease donor dependence”.

Technology developments in public health change very quickly, especially with the emergence of mHealth – there’s more mHealth programming, new applications, and emerging research.

MHealth is a key strategy for us as these applications can be used to incentivize health-seeking actions, increase the timeliness of data collection, improve patient communications, and document system-client interactions. MHealth can also facilitate workforce development through task shifting, performance support, and human resources management.

According the blog Tech in Asia, “For every 100 Vietnamese people, there’s 145 mobile phones. For a country whose population is just over 90 million, that amounts to more than 130 million mobile phones.” RefWorld.org reported that, as of January 2012, census data indicated there were 119 million mobile users in Vietnam when the population was at 88 million. Given the emergence of the mHealth industry and the large percentage of the Vietnamese population with cell phones, we at FHI 360 need to effectively mobilize this ubiquitous technology for impactful programming that helps individuals in all areas of the country protect their health and well-being.

A Social Online Learning Solution

In 2012, I first participated in TechChange’s 4-week online certificate course called “Mobile Phones for Public Health.”  I decided to take the course again in 2013 – this time with numerous colleagues — to share our mHealth programming experiences and to continue to learn from renowned mHealth practitioners around the world.

Like all busy development professionals, it is difficult to find time to cultivate learning during our day-to-day work.  The TechChange course was structured and delivered to meet our needs.

Cutting-edge, timely, and relevant information

  1.  TechChange updated its courses at least once every 3-6 months, based on direct feedback (through crowdsourcing and surveys) from its broad learning community to deliver the most up to date and relevant course content.

Great format for busy working professionals in Vietnam and beyond

  1.  Keeping our busy schedules in mind, the course content was designed to be mobile and tablet-friendly, allowing us to learn wherever and whenever fit our schedules. All live events were recorded so that learners could access materials according to their schedules.
  2. For those of us who had difficulty finishing the course in the one-month period, access (and technical support) is available for four months after the end of the course so that we can complete our final project and receive the formal certificate.
  3. The online interface was the most intuitive learner platform we have ever used:  An online course map visually illustrated all components of the program, while a calendar highlighted a variety of live discussion events with renowned experts from around the world.
  4. The main facilitator actively participated in all discussion boards; introduced weekly themes (through video, email and platform) and summarized (through print and video) the highlights of each week.  He and a facilitation team also provided “office hours” for those who needed extra support (and this support was provided in various time zones).

Interactive learning experience

  1. There was great communication between facilitator and learners. The course required 7-9 hours of effort per week and the 50 or so participants were motivated to actively participate. Learner outcomes were clearly defined and each week’s themes were well-articulated so that we knew what to expect and what was expected from us.
  2. Instead of relying primarily on print materials, sharing video, audio and weekly live events/”chats” allowed learners from various cultural backgrounds to gain knowledge and skills through a variety of channels through interactive learning.
  3. Practical exercises and interactive simulations ensured knowledge application and exchange.
  4. Individuals got to know each other through a variety of “get to know you” activities and collaborative exercises.
  5. TechChange added some fun by integrating game dynamics into the course, awarding points each time a person participated in a discussion or attended an event, with a minimum participation threshold established in order to earn a certificate.

Joining a professional network and community

  1. All learners also became TechChange alumni upon finishing the course. We are now connected through social media with others in mHealth (and offered substantial discounts on upcoming courses).
  2. It’s been great to see other mHealth alumni like Lauren Bailey making strides in the field after joining this course.

Results of mHealth Training with TechChange

Through this mHealth course, my team learned best practices as a group to explore ways to implement mHealth projects. My colleagues learned how mobile technologies are being used in other countries and sectors and thought about ways it could be applied in Vietnam. The individuals who have participated in the TechChange course are now our office mHealth champions and are actively identifying areas of work where mHealth solutions can be applied.

Here are two current mHealth pilots we have launched in Vietnam through the USAID/SMART TA program to address HIV challenges:

1.  Fansipan Challenge – uses the metaphor of Fansipan mountain (Vietnam’s highest peak), gamification, and mobile technologies to support people who inject drugs and their intimate partners to test for HIV and be linked to care if they are positive.

2.  mCare – is the first case management application in Vietnam that utilizes mobile technologies to support and track clients across the cascade of HIV outreach, testing, care and treatment services.  It also manages performance-based incentives for hamlet health workers who identify potential clients, encourage them to test for HIV, and support them to enroll and be re-engaged in HIV care and treatment and methadone maintenance treatment.

The Results of the mHealth Pilots So Far

While mCare is in its early stages of deployment and refinement, the Fansipan Challenge has shown a dramatic reduction in programmatic unit costs, combined with significant increases in testing uptake and HIV yield among underserved key populations.  Between June – November 2013, 62% of 656 injecting drug users and their intimate partners tested for HIV after a single contact. Approximately 71% of these individuals were first time testers; 17.8% were diagnosed as HIV positive. Comparative expenditure analyses of USAID/SMART TA-supported outreach services show a 50% reduction in costs associated with identifying an HIV positive person.  And preliminary data further suggest that HIV positive people identified through Fansipan have higher CD4 levels (average 287.5 cells/mm3) and will thereby have better treatment outcomes than those who initiate treatment when they are severely immuno-compromised.

These new initiatives rely on mobile technologies and we, like other technical assistance providers in the development sector, need to be constantly learning about mHealth innovations, and emerging knowledge and applications.  The TechChange mHealth class was a great investment in having my team become more familiar with mHealth as we introduced our mHealth initiatives.

Meet Carolyn Florey, this week’s featured TechChange alumna who we interviewed this week in her office at the World Bank. A serial TechChange participant of over five courses to date, Carolyn started taking TechChange courses in 2011 to supplement her Master’s program at Johns Hopkins SAIS, and has since been coming back for more continuing education in her ICT4D career.

Read Carolyn’s story below:

What interested you in taking TechChange courses? As a graduate student of International Development at Johns Hopkins SAIS, I wanted to focus on tech innovation in developing countries. I had taken a course at SAIS on the Politics of Humanitarian Aid that touched a bit on the use of technology for crisis mapping and disaster relief and wanted to learn more. I enrolled in TechChange’s TC103: Tech Tools and Skills for Emergency Management and was hooked. From there, I crafted an independent study around the format and content of TechChange courses.

What did you find useful from TechChange courses that keeps you coming back? Based on my experience taking TechChange courses in Mobiles in Development, mHealth, mData, mAccess, and Digital Organizing and Open Government, I continue taking TechChange courses for professional development and networking in my industry. Here’s why:

  1. TechChange is at the pulse of ICT4D. TechChange is great at weaving together the application of technology across sectors and industries. You always need to keep up with what’s happening, especially in this field. They’re always updating the syllabi to share the latest industry information and inviting the most relevant industry experts as guest speakers. I found that TechChange always offered courses with both breadth and depth. For example, TC105: Mobiles in International Development was a good introduction for cross-sector applications of different mobile tools, while TC309 on mHealth let me narrow my focus to relevant topics for my daily work in the Global Health Bureau at USAID.

  2. Course format and flexibility. In graduate school, I was working 30 hours a week and going to school full time with a five-course workload, so I needed the flexibility that TechChange gave to supplement my curriculum at SAIS. In my independent study, TechChange courses were helpful resources when I wrote papers and organized events for the SAIS Careers in Development club. The format and content is convenient to access as live events are recorded and archived, and course materials are available even 4 months after a course ends.

  3. Learning industry lingo and key players. One valuable take away from TechChange courses is learning key terms and industry influencers to build and demonstrate expertise in ICT4D. For example, the mHealth content I was exposed to in TC105 came through in my interview for my former role at USAID, and I think ultimately helped me get the job.

What impact has TC105 had on you and your career? TechChange is part of my regular continuing education to stay up-to-date with the most relevant information on the ICT4D industry. I’ve also made some very meaningful connections through my TechChange courses. At this point in my career, TechChange has evolved for me to become a valuable networking tool. One example of this was the TechChange happy hour during the mHealth Summit in Washington, DC last December that brought together participants of the mHealth TC309 course and attendees of the Summit.

What advice would you give to students taking TC105 or any TC course?

  1. Make attending live events a priority. Look at the live event discussions as part of your continuing education. Rarely will you get an hour of access to these industry experts.

  2. Focus on what you’re most interested in. If you’re juggling work, a TechChange course, and sometimes other courses as well, all the information can get overwhelming. Make sure to review the TechChange course syllabus ahead of time and focus on the things you’re interested in, prioritizing one week’s content over another if necessary.

  3. Read through other participants’ comments and questions. Often, other participants will have experience you don’t, so they’ll have some informed questions and insightful comments.

Prior to joining WBI, Carolyn worked as a Private Sector Partnerships Specialist at USAID, focusing on mobile technology and health partnerships. She has worked in the ICT for development space for the past few years at organizations such as Jhpiego, NetHope, the Federal Communications Commission, and Earth Institute across sectors including mHealth, mobile money, women’s access and eAgriculture. On February 26, USAID received the “Best Government Policy for Mobile Development” award at GSMA’s Mobile World Congress 2013. And while the Mobile Solutions team was receiving an award in Barcelona, TechChange and the MS team were also receiving over 1,500 mobile poll responses from recipients in DRC taking part in an online exercise designed by 173 USAID staff and implementing partners in 21 countries. The way this was possible is through harnessing the same potential for public-private partnerships used for external implementation and applying it to internal education and collaboration at USAID.

The exercise was part of a 4-week online course in Mobile Data Solutions designed to provide a highly interactive training session for USAID mission staff and its implementing partners to share best practices, engage with prominent technologists, and get their hands on the latest tool. Rather than simply simulating mobile data tools, USAID staff ran a live exercise in DRC where they came up with 10 questions, target regions, and desired audience. The intent was to not teach a tool-centric approach, but instead begin with a tech-enabled approach to project design and implementation, with an understanding of mobile data for analysis, visualization, and sharing.

This would have been a formidable exercise for any organization, but fortunately we augmented USAID’s development capacity with the abilities of three organizations. TechChange provided the online learning space, facilitation, and interactive discussions. GeoPoll ran the survey itself using their custom mobile polling tool. And MapBox provided the analysis and visualization needed to turn massive data into a simple and attractive interface. (Want to check out the data for yourself? But while the creation of an interactive online workshop for small-group interaction requires barriers to scale, the content is under no such restrictions. One of the videos from our previous course on Accelerating Mobile Money provided an animated history of M-PESA, the successful mobile money transfer program in Kenya, which allows everything mobile phone users to pay for everything from school fees to utility bills and is proving transformative in cases such as Haiti.

But there’s still plenty of work to do. As mobile phones continue their spread to ubiquity, the challenges for applying their potential to development will only increase, along with the continuing possibilities as the technology continues to improve. However, in the short term, we’re focused on increasing mobile access, which is the topic of our next course. How can USAID use mobile technologies to more effectively collect, analyze and share data?  These are the central questions we will be addressing as part of a new course TechChange has developed in partnership with the Mobile Solutions team at USAID and QED.

USAID, together with its partners, has the opportunity to increase efficiency, improve the quality of the information its uses, and better meet USAID goals related to its Forward Reforms, Evaluation Policy, and Open Data Initiative by utilizing mobile technologies to collect and disseminate data about people, projects, and programs. This course will help USAID Missions and implementing partners understand how to do just that.

Building off of the success of our 8-week online certificate course this fall on Accelerating Mobile Money, TC311 Mobile Data Solutions will be a four week online course (February 1-March 1, 2013) designed to build the necessary technical capacity to deploy mobile data collection strategies by bringing together Mission staff and implementing partners. The four weeks are structured as follows to provide a comprehensive overview of mobile devices in data collection.

Week 1: Introduction to mobile data solutions

  • What is mobile data? What are the benefits and challenges associated with collecting data wirelessly?

Week 2: Project design

  • Designing projects and preparing concept notes, scopes of work, other documents to include mobile technologies.

Week 3: Implementation

  • Study design and programming, training, field operations, data management

Week 4: Analysis, visualization and sharing

  • Utilizing data for decision-making, sharing with partners

The course will go beyond explaining the benefits of this approach. Participants will learn the questions to ask in order to assess projects (Are mobile technologies appropriate?); design them to achieve the maximum benefits possible (How should interventions be designed to take advantage of these technologies?), implement them (What device should we use? How do we train staff? What resources do we need in the field? At the Mission?), and report and share the data (How do we create visuals that can inform decision making? How do we share the results with beneficiaries and partners in-country?).

Featured tools, organizations and projects include: Episurveyor/Magpi, FormhubSouktel, EMIT, uReport, TexttoChange, RapidSMS, GeoPoll, iFormbuilder, PoiMapper, Catholic Relief Services, DAI, NASA, OpenDataKit at UW, SweetLab, JSI, ICF International, Tangerine at RTI, Futures Group. The course will be delivered on TechChange‘s custom learning platform and will include a mixture of presentations by experts, tool demonstrations, selected readings, and activities including designing and analysing a survey using mobile software.

The end of the year is now upon us. We just wanted to thank you from the bottom of our hearts and the top of our DC nerd attic for making 2012 our best one yet. Specifically, thanks to your course feedback, content contributions, happy hour attendance, and tuition dollars, we’ve trained over 1,400 participants in 70 countries in how to better use technology for social change.

New Online Courses:
We have expanded on our original set of courses (Emergency Management, Digital Organizing, and Mobiles for International Development) into exciting new spaces. A few courses we’d like to highlight:

  • (TC309) Mobile Phones and Public Health: Our largest open enrollment course so far, we were joined by over 100 students in 25 countries. Developed in partnership with the UN Foundation’s mHealth Alliance, we also piloted our new in-course tool simulator for D-Tree!
  • USAID Courses on Mobile Money: Through a custom course for 80 USAID mission staff in 7 countries, we’re helping build development capacity in mobile phones. Next up? Turning this course into a self-paced interactive module to scale the program.
  • TOL Journalist Training for “Reporting on Education” in E. Europe: Developed in partnership with Transitions Online (TOL), BBC, and The Guardian, we shared our platform with TOL to train 20 journalists over a 2-week period. This was our first course ever with non-TechChange content and external facilitators!
  • (TC201) Ushahidi: Frameworks for Effective Platform Management: Expanding on our “Emergency Management” course, we developed this course in partnership with Ushahidi to be a scalable complement to the Universities 4 Ushahidi program (U4U).
  • (TC108) Technology, Innovation, and Social Entrepreneurship: Developed in partnership with the Amani Institute, we wanted to not just teach content, but develop more social entrepreneurs to keep pushing the field forward.

Online-Enabled Public Events:
One of our initiatives this year has been to assist our partners in reaching a larger online audience and to start thinking of all public events as online-first. Events include:

  • International Conference of Crisis Mappers at the World Bank: We worked together with Crisis Mappers to produce the first livestreaming of ICCM, which led to an additional 950 unique viewers from all over the world!
  • Connecting Grassroots to Government at the Wilson Center: Building on our work for empowering Volunteer Technical Communities (VTCs), we took live questions from the online audience during this event. This was one of multiple events at the Wilson Center, which is leading the way in online-enabled events.
  • Expert Interviews at the mHealth Summit: Since the webcast was already provided, we focused on capturing expert knowledge from the attendees and partners for the mHealth Alliance. Most fun part? Getting pictures of attendees holding their cell phones to show their personal connection with the device.

Site Upgrades and Added Features:
In addition to a few other handy features, we’ve made a few big technical upgrades to our site in the hopes of improving user experience.

  • Launched a new, responsive TechChange.org! We much of 2011 promoting mobile-first design, so it was a relief to build a fully responsive site in 2012. Try re-sizing it in your browser!
  • Animating our content voiceovers. We’ve always been big fans of RSA Animate and iheed who produce educational video content, so we tried giving it a go ourselves. What do you think? We’re hoping to do plenty more in 2013.
  • Video-for-everyone course design. We switched from Ustream to OpenTok in 2012 to try to not just talk at our classes, but have discussions with you. It’s been a bumpy ride, but we’re working at optimizing for every browser and bandwidth.

Field Training and Workshops:
Tech training cannot be done by Internet alone. Here’s a few cases where we rolled up our sleeves and got to teaching the old fashioned way.

This week will mark the launch of our new eight-week online certificate course on Mobile Money in partnership with the Mobile Solutions team at USAID and The QED Group. Today, we open our doors to over 70 staff from USAID and implementing partners in 10 countries to connect them with leading experts in the mobile money space and share best practices. Building on the materials in our TC105 Mobiles for International Development course, we will focus more specifically on how to implement and operationalize mobile money programs designed for scale.

USAID is the first bilateral donor to establish a dedicated Mobile Solutions team to help lever the power and reach of mobile technology to accelerate development outcomes. It is also an emerging leader in mobile money. Already, USAID’s work in Haiti led to more than 5 million mobile money transactions and a doubling of financial access points. In Afghanistan, USAID worked with the Central Bank to reduce regulatory barriers to entry for mobile network operators to establish mobile money products. And it is working to transfer large payment flows from cash to mobile money–this summer, for example, 110,000 households will have the chance to pay their electricity bills via mobile money. This online course will help inform this work and future programs, as USAID intends to expand its mobile money work to countries like Indonesia, Philippines, Malawi and a host of others.

Participants will explore at issues of regulation and interoperability, as well as strategies for working effectively with MNOs, banks and governments. We’ll consider case studies from countries like Pakistan, Philippines, Haiti, Uganda, Kenya and hear from guest experts such as David Porteous of Bankable Frontiers, Kabir Kumar of CGAP, Nadeem Hussain of Tameer Bank, Tomasz Smilowicz of Citigroup, Jordan Weinstock of Open Revolution, Chrissy Martin of MEDA, and more. Self-paced content including animated and interactive overview videos will complement live discussions.

According to Priya Jaisingani, Director of the Mobile Solutions team at USAID, this type of online facilitated course “will go a long way to ensure USAID’s mobile money work is informed by the best thinking in the field.”

TechChange president Nick Martin said of the partnership: “We often find that the biggest barrier to using mobile phones for international development is bridging the rift between development projects and technologists. We’re hoping this class can do exactly that through sharing best practices and connecting with leading experts.”

A couple weeks ago, Nick Kristof published a New York Times article titled DIY Foreign-Aid Revolution and Dave Algoso wrote an excellent critique just a few days later in Foreign Policy. Because this is the internet and two weeks makes it venerable news, I thought about letting it slide by, but it’s been bugging me increasingly since then. This is partly because I have worked both at a small 3 person NGO with a great idea and not much else, as well as a large USAID contractor with hundreds of staff members and millions of dollars in projects, and I still cannot decide which is “better.” But it’s also because I think ICT for development has a unique spot between these worlds, and I think it’s going to change the conversation in years to come.
